Position Responsibilities: The Resident Service Coordinator Chelmsford-Floater (RSC Chelmsford-Floater) is responsible for the direct coordination of services and supports to residents at an assigned housing site four days/week (24 hours). The RSC Chelmsford -Floater will also be available three days/week (13.5 hours) to cover vacancies/absences/vacations at AgeSpan's Supportive Housing sites. Whether at their home base or as a floater, they are responsible for identifying and accessing resources beneficial to residents as needed, acting as a liaison between the Housing Authority/Property Management and residents when appropriate, and working collaboratively with Housing Authority/Property Management staff on behalf of all residents.
Essential Functions: (Core duties or tasks that are fundamental and not marginal to the performance of the job)

<li>Work on site no less than 24 hours over four days per week, including every Friday from 9:00 a.m.-3:00 p.m. Schedule is subject to change.</li>

<li>Provide coverage as a floater at multiple housing sites within the Merrimack Valley and North Shore regions when needed, ensuring continuous support during staff vacancies or absences.</li>
<li>Travel Requirements: The position involves travel between multiple housing sites, and flexibility is essential to adapt to the varying environments and unknowns associated with different locations.</li>
<li>Work with CHA RSC to develop and implement a collaborative plan for the campus.</li>

<li>Coordinate and facilitate services/activities and schedule office hours at designated housing site(s).</li>
<li>Inform residents about State Home Care and other publicly funded programs and public benefits. Make referrals and assist residents with securing benefits as appropriate. </li>
<li>Recruit, train and supervise volunteers who will assist with group/congregate activities.</li>
<li>Identify gaps in services for residents. Develop, implement and offer a range of services and programs to residents with the goals of establishing and maintaining cost effectiveness and affordable services, and meeting unmet needs.</li>
<li>Assist in screening residents for Home Care Program eligibility and establish appropriate care plans with the AgeSpan Care Manager(s) and Nurse(s) assigned to the sites.</li>
<li>Work closely with community organizations and social service providers to provide programs, supports and services to allow residents to age in place.</li>
<li>Provide accurate and timely documentation to meet all contractual and administrative standards as required by the Department of Housing and Community Development, Elder Services and, if appropriate, the Housing Authority. Provide regular updates, feedback and reports to Housing Authority staff to preserve residents' tenancy.</li>
<li>Organize regular meetings with residents to address social, recreational and educational needs. Identify problems and issues and approaches to dealing with them.</li>
<li>Attend regularly scheduled meetings with supervisor and RSC team, and AgeSpan all staff meetings.</li>
<li>Attend relevant trainings and workshops.</li>
<li>Other related duties as assigned.</li>

Qualifications: BA/BS Degree preferred; LSW preferred. Knowledge of and experience with housing/social services with elderly/disabled population strongly preferred. Knowledge of Housing/Disability laws and regulations helpful. Solid written, verbal, interpersonal and team skills. Strong computer skills and comfort with Microsoft applications and programs. MARSCH (Massachusetts Association of Resident Service Coordinators) Certification helpful but not required. SHINE (Serving the Health Insurance Needs of Everyone) certification a plus.

Roxbury Tenants of Harvard Association, Inc. (RTH) is a private 501(c)(3) multi-service non-profit housing and human service organization founded in 1969 by Mission Hill residents and Harvard University students. Our mission is to preserve the unique character of the neighborhood by moderating the expansion of local institutions into our community.
RTH is a dynamic, racially, ethnically, and culturally diverse community that owns and operates high-quality low- and moderate-income housing. Our 24-acre site in the Mission Hill neighborhood of Boston is adjacent to the LMA. Despite being surrounded by large buildings and congested streets, our grounds offer a comfortable setting with a variety of recreational and community facilities, including an outdoor pool, computer resource center, after-school program, senior drop-in center, and a Community Center with a full gymnasium, fitness center, and youth development center. Challenges facing our residents include poverty, fragile families, isolation, unemployment, literacy issues, chronic disease, disability, mental health issues, and health barriers.
RTH housing includes four high-rise buildings and 144 townhomes in the Mission Park section of the campus, a high-rise building with 145 units on Fenwood Road (the Mosaic), and 190 apartments in Victorian homes and midrise buildings on Fenwood Road, Francis Street, St. Albans Street, and Huntington Avenue.
