Reports to the Director of Family Support
Do you like assisting others in accessing necessary resources and broadening their involvement in the community? In this role, you assist individuals with developmental disabilities and their families by providing service coordination and advocacy while linking them to existing community and state resources. This position also assists individuals with budget administration. You balance the use of informal and formal resources while maximizing family control over services. At the request and direction of the family/individual, you serve as a liaison with community, state agencies, and service providers. You understand and promote the transition process and support individuals/families through planning the transition from the educational system to adult supports/services, including natural supports and the provider selection process. You facilitate the legal process for individuals/families as necessary including Guardianship, Power of Attorney, Advance Directive planning, and Representative payee. Knowledge of benefits will be a key component of this position, and you will build a strong knowledge base of adult services and transitions from school services to adult services. You will identify and connect families to local and statewide resources and facilitate planning meetings, ISAs, amendments, and action plans. The ideal candidate will offer flexibility, be a creative problem solver and collaborative team player, and have a strong background with documentation. Experience working with families from diverse backgrounds in a non-judgmental manner is necessary and knowledge of IEPs is a plus. Home visits are required, which necessitates reliable transportation. We will train the right person.

Do you possess excellent organizational skills? Do you have at least 2 years of case management or related experience? As a Family Services Coordinator (FSC), you will assist individuals with developmental disabilities and their families by providing service coordination and advocacy while linking them to existing community and state resources. Here are a few examples of your responsibilities: You advocate and support families and individuals in a manner that increases opportunities for participation in the community by responding to the changing needs of the family. You coordinate assessments and evaluations for all individuals as assigned. You secure all necessary referrals, documentation, and relevant data to determine the individual’s unique needs including though not limited to the Support Intensity Scale (SIS) and Health Risk Screening tool (HRST). You facilitate planning meetings for individuals including completing, compiling, and processing Individual Service Agreements/Amendments/Action Plans and other related documents as requested or required. In other words, you serve as the hub in the wheel of an individual's and their families' support with PathWays. We are looking for a team player to oversee the daily operations, in conjunction with another manager and nursing staff, of a residential home ensuring quality services are provided to the individuals served. You will supervise residential support professionals and serve as the liaison for teams and meetings related to the services provided. You will need to perform direct care (including personal care such as toileting, bathing, feeding, etc.) as needed as well as ensuring that daily tasks such as laundry, cooking, etc. are completed. Specific Duties and Responsibilities:
<ul>
<li>Provide individualized type and level of supports to include, but not be limited to, direct support, personal care and/or skill acquisition etc.</li>
<li>Distribute medication in accordance with He-M 1201 as applicable.</li>
<li>Encourage and assist individuals in accessing and fostering community supports and resources such as social/cultural groups, stores, restaurants, etc.</li>
<li>Support, in a proactive manner, individuals in being a part of their community through such activities as relationship building, social interactions, exercising their roles and rights as citizens, etc.</li>
<li>Perform all duties in a manner that demonstrates an awareness of and respect for each person's legal, civil, and human rights-including maintaining confidentiality of information regarding the individual -and that ensures the respect and dignity of persons with a developmental disability</li>
<li>Provide all supports in a manner which takes into consideration the preferences of individuals and which actively encourages choice making by the individual.</li>
<li>Assist individuals in establishing, maintaining, and presenting a positive image. This may involve such activities as assisting with bathing, showering, personal and dental hygiene, etc.</li>
<li>Recruit, hire and supervise staff assigned to the residence.</li>
<li>Develop, implement and monitor service plan goals for each individual served.</li>
<li>Represent the agency in a professional manner at all times.</li>
<li>Attend the service plan meeting, team meetings, agency and staff meetings and other meeting and/or conferences as requested by supervisor.</li>
<li>Ensure compliance with applicable state regulations and agency policies and procedures (including employee time sheets, treatment plan data and reports, medication logs, attendance sheets, fire drills, and other pertinent documentation as requested).</li>
<li>Provide responsive and clinically/medically based services and supports in accordance with individual needs, desires and dreams. Such supports will promote the dignity, respect of all individuals.</li>
<li>Coordinate the involvement of family members, outside professionals and involved team members.</li>
<li>Promote and maintain satisfactory public relations between the residence, agency and community.</li>
<li>Arrange for adequate direct service staffing to all services in accordance to ISP and individual needs. Provide coverage for direct service as needed.</li>
<li>On call responsibilities in response to department needs on a rotating basis.</li>
<li>Perform additional duties and responsibilities as assigned</li>
</ul>

<li>Ability to acquire computer competencies.</li>
<li>Ability to work a flexible work schedule with occasional nights and weekends.</li>

Work Environment and Physical Demands:
<ul>
<li>Work Environment: Work environment is occasionally hectic with occasional periods of high stress. Occasional to moderate short deadlines and coverage require flexibility in schedule.</li>
<li>Physical Demands: Must be physically able to occasionally lift a minimum of 25 pounds and to provide transfers as necessary. Walking and standing required frequently with occasional stair climbing. Must be able to push wheelchairs and operate wheelchair lifts/Hoyer and barrier free lifts. Emergency behavioral intervention in accordance with behavioral intervention techniques and individual behavioral plans.</li>

Travel:
Will regularly travel (access to personal vehicle during all scheduled work hours) with any supported individual utilizing personal vehicle as well Community Choices' vehicles. Must possess and maintain a valid NH license and safe driver's record in accordance with Community Choices standards with adequate liability insurance.
