Our region consists of Sullivan and Lower Grafton County in NH and our programs work with individuals from birth and throughout their lifespan as needed. This position works out of Claremont, NH with regional travel and/or within-state travel. In addition, the position is full-time and the starting pay range is $21.00 to $23.00 per hour depending on experience.

Do you possess excellent organizational skills? Do you have at least 2 years of case management or related experience? As a Family Services Coordinator (FSC), you will assist individuals with developmental disabilities and their families by providing service coordination and advocacy while linking them to existing community and state resources. Here are a few examples of your responsibilities: You advocate and support families and individuals in a manner that increases opportunities for participation in the community by responding to the changing needs of the family. You coordinate assessments and evaluations for all individuals as assigned. You secure all necessary referrals, documentation, and relevant data to determine the individual’s unique needs including though not limited to the Support Intensity Scale (SIS) and Health Risk Screening tool (HRST). You facilitate planning meetings for individuals including completing, compiling, and processing Individual Service Agreements/Amendments/Action Plans and other related documents as requested or required. In other words, you serve as the hub in the wheel of an individual's and their families' support with PathWays. This position does require travel throughout the state of NH and mileage is reimbursed. 
 
This position requires:
<ul>
<li>A Bachelor's degree is highly preferred; extensive Case Management experience will be considered in place of educational credentials</li>
<li>2+ years of Case Management or closely related work is required</li>
<li>A minimum of 2 years of human services experience is a must with experience with developmental disabilities highly desired</li>
<li>Excellent verbal, written, and interpersonal communication skills are a must.</li>
<li>Minimum of intermediate proficiency with MS Office (Word, Excel, Outlook) is required and the ability/willingness to learn new programs is a must.</li>
<li>Willingness and ability to travel either regionally or within the state of NH are a must. Access to reliable transportation is required.</li>
