\n\nRegistered Nurse Qualifications\n

  • Active RN license in Vermont and New Hampshire or compact nursing license\n
  • Graduate of an accredited nursing program\n
  • Minimum 1 year of clinical nursing experience (Medical/Surgical, Oncology, or Hospice RN experience preferred)\n
  • Strong communication and organizational skills\n
  • Ability to work independently and collaboratively within an interdisciplinary healthcare team\n

\n\nHospice RN Responsibilities\n

    \n
  • Develop individualized patient care plans in collaboration with physicians, patients, and families/caregivers\n
  • Coordinate and communicate hospice care needs with the interdisciplinary healthcare team\n
  • Conduct ongoing assessments of patients’ clinical, functional, and emotional status\n
  • Document patient evaluations, responses, and progress toward care goals\n
  • Educate patients and caregivers on hospice care techniques and end-of-life symptom management\n
  • Participate in case conferences and recertify hospice care plans as needed\n
  • Ensure compassionate pain management and comfort for patients with terminal or chronic illness\n
