\n\nRegistered Nurse Qualifications\n
- \n
- Active RN license in Vermont and New Hampshire or compact nursing license\n
- Graduate of an accredited nursing program\n
- Minimum 1 year of clinical nursing experience (Medical/Surgical, Oncology, or Hospice RN experience preferred)\n
- Strong communication and organizational skills\n
- Ability to work independently and collaboratively within an interdisciplinary healthcare team\n
\n\nHospice RN Responsibilities\n

- Develop individualized patient care plans in collaboration with physicians, patients, and families/caregivers\n
- Coordinate and communicate hospice care needs with the interdisciplinary healthcare team\n
- Conduct ongoing assessments of patients’ clinical, functional, and emotional status\n
- Document patient evaluations, responses, and progress toward care goals\n
- Educate patients and caregivers on hospice care techniques and end-of-life symptom management\n
- Participate in case conferences and recertify hospice care plans as needed\n
- Ensure compassionate pain management and comfort for patients with terminal or chronic illness\n
