Duties include, but are not limited to the following:
Assist individuals with intellectual, developmental and other disabilities to build and maintain their ability to participate in community activities by focusing on skill areas that include communication, self-care, relationship-building and community involvement. As Case Manager work closely with the Program Director, Assistant Director, CBDS Coordinators, and other members of each person’s ISP (Individual Support Plan) team in the development and implementation of all Individual Support Plans. Implement the ISP objectives and documenting participants' progress by maintaining accurate, thorough, and up-to-date individual case records for each participant assigned to case load and attend all ISP meetings, annual ISP meeting and other related meetings as needed. Document all assigned written materials (progress notes, behavior plan data, access database entries, communication log notes, etc.) in a clear, objective and timely manner. Implement Positive Behavior Supports (PBS) by providing necessary interventions, as outlined in individuals’ plans and as directed by the Program Director and/or PBS Coordinator Assess participant's interests, preferences, and abilities by providing access and opportunity to explore personal and community possibilities in a way that supports informed choice. Coordinate all services received and required by the participants on their assigned caseload. Identify participants needs, learning styles, intervention, time frames, assignments and follow-ups. Communicate and establish professional relationships with participants, their family, guardians, direct care supervisors, residence staff, service coordinators, and consultants on a regular basis, within established guidelines, documenting all significant contact. Provide proper supervision and be familiar with medical protocols (ie. seizures, diabetes, allergies), emergency interventions, evacuation routes, fire extinguisher &amp; first-aid kit locations. Valid Driver's License with satisfactory driving record. Satisfactory background check, including CORI, SORI and fingerprinting required
