\nWork Hours: NIGHTS – 12HR: 7p-7a must be available on weekends; BLOCKED BOOKING NOT ALLOWED\n\nTotal Weekly Hours: 36\n\n \n\n \n\nDescription\n\n\nLOCATION – Sobriety Centers of NH\n\nShift: 7p-7a; must be available on weekdays as well as weekends\n\nAddress: 55 Main Street Antrim, NH 03440\n\n*Must be okay floating to their new Tilton location when it opens* \n\nPosition: RN\n\nDepartment Details:\n\nThis is a Residential Treatment Center with Detox beds for adults. They have 9-12 Detox beds and 23-36 Residential beds that can be flexed up to 35. Day shift there are 1-2 nurses, typically 2 during med pass and night shift there is only 1 nurse on with 1-2 BHA’s for support. Patients have been medically cleared prior to admission, there are no incarcerated individuals and all patients are voluntary.\n\n \n\nDayshift has 2-3 BHAs, 2+ clinical and admin; Swing has 2-3 BHAs and 1 clinical on late date and Night has 1-2 BHAs and one RN; but there is always an on-call admin as well. Orientation is 3-5 days. Interviews will be conducted via TEAMS with the camera on.\n\n The Registered Nurse is responsible for providing nursing care to patients. Supervising care treatment and services of patient care provided by LPNs, CNAs and other staff on assigned unit and shift. Developing, implementing, and evaluating an individual plan of care for each patient. Assessment and re-assessment of patients. Communicating appropriate information with the interdisciplinary team. Reporting patient concerns and changes in condition to supervisor and/or physician in a timely manner.\n\nRequired Certifications: RN license, BLS, CPI (not offered on-site)\n\nRequired Experience: 2 years of psychiatric nursing experience in an inpatient setting\n\nDress Code: Scubs\n\nDocumentation: EHR-HCS\n\nAdditional Information:\n\nWe are looking for an individual who has strong boundaries and work ethic that provides compassionate care to those struggling with SUD. We are open to all pathways to recovery and support those on MAT including methadone. Ideal candidate is punctual, professional, caring and ready to jump in and support both clients and staff.\n\n \n\n· Provides and coordinates care by assessing physical and behavioral health needs of patient, develops and implements nursing care plans, maintains medical records and educates patients and their families about various physical and behavioral health conditions while ensuring patient safety at all times.\n\n· Assesses patient’s status on an ongoing basis and pertinent information gathered is documented.\n\n· Ensures nursing treatment plans are specific to assessed needs and initial treatment plan is completed upon admission.\n\n· Takes physician’s orders (verbal or via telephone) accurately and documents appropriately; transcribes and reviews physician’s orders. Ensures physician’s orders are legible if taken verbally and transcribed appropriately per policy and procedure.\n\n· Accurately administers medications per ordered note and documents accurate count of controlled substance. Adjusts medications under provider’s supervision as needed; monitors lab results to validate medication compliance.\n\n· Conducts psychoeducation and life skills groups as needed.\n\n· Notifies physician of significant changes in status such as difficulties with detox, medical emergency or change in medical status regarding response to medications, etc.\n\n· Supervises self-administration of medications using safety initiative of using two means of patient identification: documents and reports effects of scheduled and PRN medications.\n\n· Maintains accountability of administration, recording, control and security of medications, including narcotics.\n\n· Completes admission and discharge processes efficiently and accurately as directed.\n\n
