Job Summary:
We are seeking a highly qualified and compassionate LPN for our Home Health Aide Supervisor position at Abernathy Home Care in Stoneham, Massachusetts. This is a part-time, hourly healthcare/medical job where the individual will provide supervision to our team of home health aides. The ideal candidate will have a current LPN license and be committed to providing exceptional care to our clients in the comfort of their own homes.
Compensation & Benefits:
The pay for this position is $35 to $39 per hour plus mileage and travel time.
Responsibilities:
- Provide direct supervision and support to home health aides, including training, monitoring, and evaluating their performance
- Collaborate with the care team to develop and implement individualized care plans for clients
- Conduct initial and ongoing assessments of clients' health status and communicate any changes or concerns to the appropriate healthcare professionals
-- Follow infection control and safety protocols to maintain a safe and clean environment for clients and staff
- Communicate effectively with clients, family members, and healthcare professionals to ensure quality care and positive outcomes
- Maintain accurate and up-to-date documentation on client care and progress
- Adhere to all state and federal regulations, as well as company policies and procedures
Requirements:
- Current LPN license in Massachusetts
- Minimum of 1 year experience in a healthcare setting, preferably in home health or geriatrics
- Strong clinical skills and judgment
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Reliable transportation and a valid driver's license
- Willingness to travel to client's homes
- Flexibility to work evenings and weekends as needed
