SKILLED NURSING FACILITY:

The Skilled Nursing Clinical Liaison serves as a key connector between Community Health providers, skilled nursing facility (SNF) staff, and patients. This role blends clinical nursing skills with administrative coordination to ensure high-quality, efficient, and patient-centered care for Community Health patients residing in nursing homes. The liaison supports the provider team, facilitates transitions of care, maintains accurate medical records, and ensures compliance with regulatory and organizational protocols.

This is a fast-paced, dynamic role suited for a licensed LPN or experienced Medical Assistant (MA) with a passion for geriatric care, strong communication skills, and comfort navigating electronic health records (EHRs).

ABOUT THE ROLE:

The Community Health LPN Skilled Nursing Clinical Support will assist the providers in managing an optimal workflow by providing a variety of nursing and administrative duties to provide safe and efficient care of the patients.

FUNCTIONS OF THE POSITION:

  • Obtain and document vital signs using multiple EHR systems.
  • Perform routine nursing procedures (e.g., EKGs).
  • Reconcile medication lists during rounds and care transitions.
  • Assist providers with reordering medications and DME supplies.
  • Coordinate lab orders, specialty consults, and imaging studies.
  • Participate in discharge planning and complete transition templates for handoff to Care Managers.
  • Serve as the central communication point between Community Health providers and SNF nursing teams.
  • Notify SNF staff of new orders and ensure understanding of clinical indications.
  • Track patient status changes (e.g., admissions, discharges, room transfers).
  • Maintain up-to-date provider rounding and on-call schedules.
  • Support Care Managers with discharge planning and follow-up care.
  • Schedule and track all regulatory visits and required documentation.
  • Maintain accurate payer and insurance data in collaboration with billing staff.
  • Ensure EMR reflects real-time updates on patient status and transitions.
  • Order clinic supplies and coordinate maintenance of clinical equipment.
  • Field incoming calls from SNF staff, triaging needs and escalating as appropriate.
  • Document nursing home metrics and encounter summaries in EMR.
  • Ensure timely delivery of provider notes to SNF and outside providers.
  • Review daily on-call reports to support the continuity of care.
  • Build strong relationships with SNF staff and act as a professional representative of Community Health.
  • Participate in team huddles, quality improvement initiatives, and care planning meetings.
  • Foster a culture of respect, accountability, and excellence in care delivery.

SKILLS REQUIRED FOR SUCCESS:

  • Current Vermont LPN license (or can obtain upon offer of employment)
  • Prior experience working in a nursing home/ geriatrics preferred.
  • Experience in using a variety of electronic medical records and ability to learn other systems, basic keyboarding skills and email communication.
  • Strong interpersonal skills and ability to work independently
  • Experience with EMR systems and general computer literacy
