Clinical Manager:

As our Clinical Manager, you will:

  • Lead and empowerour dedicated field staff (nurses, social worker, and chaplain) in our home care and hospice programs.

Key Responsibilities:

  • Performance Management & Development:Conduct annual performance reviews and field supervision assessments, coordinate staff schedules and time off, and oversee clinical orientation for new field staff.
  • Patient Care Oversight:Ensure high-quality patient care through collaboration on patient and personnel assignments, care coordination, referral guidance, continuous needs assessment, and the development/implementation/updates of individualized plans of care.
  • Team Leadership & Management:Provide day-to-day management and coordination of field staff, fostering consistency in treatments and documentation.
  • Quality & Compliance:Assist with quality management initiatives, participate in utilization review, and work with the Quality Director to track key program indicators and meet national averages. Maintain current knowledge of professional practice and regulatory requirements (including Medicare Conditions of Participation).
  • Patient Advocacy & Service Excellence:Respond to patient complaints with a focus on improvement and deliver patient care with exceptional service.
  • Operational Efficiency:Manage tasks accurately and efficiently, monitor productivity and documentation, assist with skilled visits and on-call rotation as needed, and contribute to efficient scheduling and resource utilization.
  • Interdisciplinary Collaboration:Facilitate interdisciplinary group (IDG) meetings and ensure effective communication across the care team.
  • Training & Mentorship:Set up and monitor training for new field staff, provide ongoing coaching to ensure compliance and address performance needs.
  • Administrative Support:Assist with EMR tasks (order entry, medication input), reconcile timecards, manage timecard approvals and overtime, and participate in the recruitment process.
  • Resource Management:Monitor and disseminate medical supplies, ensuring accurate EMR entry.

Qualifications We Are Looking For:

  • Required:Associates degree in nursing.
  • Preferred:Bachelors degree in nursing.
  • Essential:CurrentNew Hampshire RN license in good standing.
  • Experience:Previous management experience preferred.
  • Communication:Fluent in written and spoken English.
  • Availability:Willingness to work Saturday and Sunday if needed.
  • Driving:Valid drivers license without restrictions and proof of car insurance ($100,000/$300,000 coverage).
  • Compliance:Ability to pass required background checks (criminal, driving, Bureau of Elderly and Adult Services) and provide proof of eligibility to work in the U.S.
  • Physical Requirements:Ability to sit for extended periods, view small print, and meet the lifting requirements of the position. Comfortable with occasional in-home patient visits and travel.
