Physical Therapist (PT) – LTC/SNF - Travel Position
Location: Pittsfield, MA
Job Type: Travel Contract
Duration: 13 weeks
Shift: 3x12s
Pay: $2,055/week
Job Description:
We are seeking a compassionate and skilled Physical Therapist (PT) to join a skilled nursing and long-term care facility in Pittsfield, MA, on a travel contract. The PT will evaluate residents, develop individualized treatment plans, and deliver therapy services to enhance mobility, independence, and quality of life. The ideal candidate is patient-centered, organized, and experienced in geriatric rehabilitation.
Key Responsibilities:
- Perform patient evaluations to assess mobility, strength, and rehabilitation needs.
- Develop and implement individualized treatment plans in collaboration with the care team.
- Provide therapeutic interventions including strengthening, gait training, balance activities, and mobility programs.
- Educate residents, families, and caregivers on therapy goals, safety practices, and home exercise programs.
- Document evaluations, progress notes, and outcomes accurately and timely.
- Collaborate with interdisciplinary healthcare staff to ensure comprehensive and effective care.
- Promote resident safety, independence, and quality of life through evidence-based therapy.
Qualifications:
- Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy from an accredited program.
- Current and valid Physical Therapist license in Massachusetts (or compact license).
- Minimum of 1 year of PT experience, preferably in long-term care or skilled nursing facilities.
- Current Basic Life Support (BLS) certification.
- COVID vaccine and recent TB test.
- Strong communication, organizational, and clinical assessment skills.
- Compassionate approach with experience working with geriatric patients.
