Job Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will have experience in various nursing settings, including assisted living and urgent care, and possess strong skills in pain management and memory care. As an LPN, you will play a vital role in providing high-quality patient care, assisting with medical imaging, and supporting discharge planning processes. Your expertise will contribute significantly to the overall well-being of our patients.

Responsibilities

  • Provide direct patient care under the supervision of registered nurses and physicians.
  • Administer medications and treatments as prescribed, including IV infusions.
  • Monitor patients' vital signs and report any changes to the healthcare team.
  • Assist with pain management strategies and support patients in memory care programs.
  • Collaborate with multidisciplinary teams to develop and implement individualized care plans.
  • Participate in discharge planning to ensure a smooth transition for patients leaving the facility.
  • Maintain accurate patient records using electronic health record systems such as Athenahealth.
  • Educate patients and their families about health conditions, treatment options, and self-care techniques.

Requirements

  • Valid Licensed Practical Nurse (LPN) license in the state of practice.
  • Experience in assisted living, urgent care, or Level I Trauma Center preferred.
  • Proficient in pain management techniques and memory care best practices.
  • Familiarity with medical imaging procedures is a plus.
  • Strong communication skills and ability to work collaboratively within a healthcare team.
  • Compassionate approach to patient care with a focus on empathy and respect for individuals' needs.
  • Ability to adapt to fast-paced environments while maintaining attention to detail.
