Oakdale Rehab and Skilled Nursing Center is looking for a Quality Service Coordinator on a full-time (40 hours) basis. The Quality Assurance Coordinator is responsible for supporting high standards of care through clinical oversight, quality monitoring, and interdepartmental collaboration. This role focuses on wound management, infection control support, quality improvement initiatives, and staffing coordination. The ideal candidate is a clinical professional with strong organizational and communication skills who can effectively support nursing leadership and contribute to a resident-centered care environment.

Pay Range (depending on experience): $38.00 - 44.00/hr

Key Responsibilities:

Clinical Oversight & Quality Assurance

  • Conduct and document wound rounds; ensure accurate wound tracking and timely interventions.
  • Support the Infection Control Nurse with PPE compliance monitoring and staff education.
  • Identify opportunities for quality improvement and participate in Quality Assurance and Performance Improvement (QAPI) meetings and initiatives.
  • Conduct frequent unit rounds to evaluate resident status, fall prevention strategies, and overall care quality.
  • Monitor direct care activities to ensure compliance with facility policies and procedures.
  • Provide direct resident care as needed.

Care Planning & Resident Coordination

  • Ensure that resident assessments and individualized care plans are developed, implemented, and reviewed/revised regularly.
  • Coordinate and communicate effectively with residents, families, and healthcare providers.
  • Assist with booking appointments and coordinating transportation for resident care needs.

Administrative & Leadership Support

  • Assist with recruitment, onboarding, and retention strategies for nursing department staff.
  • Fill in for Unit Managers and/or Case Managers as needed, providing leadership and continuity of care.
  • Maintain positive, collaborative working relationships between nursing staff and other facility departments.
  • Participate in the weekend and holiday on-call rotation to support 24/7 nursing operations.

Qualifications:

Education: Licensed RN or LPN in good standing with the state Board of Nursing.

Experience: Minimum of 2–3 years in long-term care or skilled nursing facility preferred. Prior QA/QI or leadership experience strongly desired.

Skills:

  • Strong clinical assessment and documentation skills
  • Knowledge of wound care, infection control, and quality improvement processes
  • Ability to lead interdisciplinary teams and coordinate care across departments
  • Excellent communication, problem-solving, and organizational skills
  • Proficient in EMR systems and Microsoft Office (Word, Excel, Outlook)
