Position Summary:
The Case Management Supervisor provides direct supervision to a team of case managers as assigned while maintaining a small caseload to stay current with all case management requirements.
Essential Functions:
All Case Management Supervisor activities involving the individual may be inclusive of family members, guardians, and/or other members of a support team, dependent upon legal relationships and individual situations and needs. The Case Manager Supervisors responsibilities include the following:
- Supervise the work of an assigned team of Case Managers.
- Complete a Brain Injury Fundamentals Training.
- Complete training on the Brain Injury Program guidelines and the TBI Act of 1996 and reauthorizations.
- Review and approve Individual Service Assessments (ISA).
- Review and approve documentation notes in the individual record.
- Ensure case managers meet regulatory and company deadlines for the ISAs, notes, required training, etc.
- Conduct monthly 1:1 supervisory meeting with Case Managers.
- Monitor and ensure case managers comply with company policies and procedures and all applicable state/federal laws and regulations related to providing Case Management for waiver participants.
- Implement company progressive discipline as needed.
- Maintain a small caseload and provide caseload coverage as needed when a Case Manager is on leave.
- Complete monthly scorecards on each case manager.
- Provide assistance in keeping individuals Medicaid eligibility current.
- Other duties as assigned by the State Program Administrator.
Position Type/Expected Hours of Work:
This is a full-time position. Days and hours of work are Monday through Friday, 8:00am to 5:00pm. Occasional evening and weekend work may be required as job duties demand.
Travel:
Travel is required. Travel can be local during the business day, and some out-of-area and overnight travel may be expected.
Required Experience:
- Each Case Manager Supervisor must have knowledge of Vermont’s public service system for developmental disabilities services and brain injury services and at least:
- At least two years of Case Management experience provided in Vermont to people with developmental disabilities and/or brain injury and meet Qualified Developmental Disability Professional (QDDP) qualifications for at least one year.
- An associate degree in human services or a closely related field and two years of human services related experience, such as supporting individuals with disabilities, special education, sociology, vocational rehabilitation and/or employment, protection and advocacy, psychology, substance use, aging, healthcare, social work or another related field; or three years of human services related experience, such as work providing assistance to people and groups with issues, such as supporting individuals with disabilities, special education, sociology, vocational rehabilitation and/or employment, protection and advocacy, psychology, substance use, aging, healthcare, social work or other related field.
Preferred:
- A bachelor’s degree in human services, or a closely related field; or
- A bachelor’s degree in any field and one year of human services related experience such as supporting individuals with disabilities, special education, sociology, vocational rehabilitation and/or employment, protection and advocacy, psychology, substance use, aging, healthcare, social work, or other related field.
- A registered nurse or a social worker with a graduate degree from a school of social work accredited or approved by the Council on Social Work Education or other comparable body; OR with a Bachelor of Social Work degree from a college or university accredited or approved by the Council on Social Work Education or other comparable body.
- Must reside in Vermont.
