Position Overview
The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a Long-Term Care (LTC) setting. This role works collaboratively with Registered Nurses, physicians, and interdisciplinary care team members to deliver safe, high-quality, and resident-centered care in compliance with organizational policies, state regulations, and safety standards.
Key Responsibilities

Administer medications and prescribed treatments in accordance with physician orders

Monitor, assess, and document resident conditions and responses to care

Assist residents with activities of daily living (ADLs)

Collaborate with Registered Nurses, physicians, and interdisciplinary care team members

Identify and promptly respond to changes in resident condition and report appropriately

Follow individualized care plans and established protocols

Maintain compliance with state regulations, safety standards, and organizational policies
Experience Requirements
-
Minimum of 2 years of Long-Term Care (LTC) and/or Rehabilitation experience required
Certifications & Licensure
-
Active Licensed Practical Nurse (LPN) license (state-specific and in good standing)

Current BLS/CPR certification
Required Skills
-
Strong clinical judgment, communication, and documentation skills

Ability to work a full 12-hour day shift and meet contract requirements
