Position Summary
We are seeking a Speech Therapist / Speech Language Pathologist (SLP) to join our interdisciplinary care team. In this role, you will provide skilled speech and language therapy services to patients in their homes, focusing on improving communication, swallowing, and overall quality of life. You will work closely with nurses, therapists, and other care providers to develop individualized care plans tailored to each patients needs.
Responsibilities
- Conduct comprehensive evaluations of speech, language, communication, and swallowing disorders.
- Develop and implement individualized treatment plans to address patient goals.
- Provide therapy for patients with neurological conditions, post-stroke complications, voice disorders, or other impairments.
- Educate patients, caregivers, and families on therapy techniques, exercises, and communication strategies.
- Collaborate with the interdisciplinary team to ensure holistic, patient-centered care.
- Document clinical findings, progress notes, and treatment outcomes in accordance with PBH&HH standards and regulatory requirements.
Qualifications
- Masters degree in Speech-Language Pathology from an accredited program.
- Current NH licensure as a Speech Language Pathologist (or eligibility).
- Previous experience in home health, hospice, or related healthcare setting strongly preferred.
- Strong interpersonal, organizational, and communication skills.
- Ability to work independently while being a collaborative team player.
