- End Date: March 28, 2026\n
- Contract Duration: 13 weeks\n
- Shift: 8:00 AM – 4:30 PM (8 Hour Days)\n
- Hours per Week: 40\n
- Weekend Rotation: Once every 6 weeks\n
\n\nRegistered Nurse Qualifications\n

- Active Massachusetts RN license\n
- At least 1 year of certified Home Care nursing experience\n
- Minimum 1 year of Case Management experience in a clinical or home health setting\n
- Ability to travel to patient homes across Newton, Lynn, Nahant, and Saugus\n
- Strong assessment, care planning, and clinical documentation skills\n
- Proficiency with electronic medical records (EMR)\n
\n\nHomecare RN Responsibilities\n

- Assess patient conditions and develop individualized home health care plans\n
- Deliver direct nursing care, patient education, and family support in home settings\n
- Perform admissions and manage cases according to Massachusetts healthcare regulations and organizational policies\n
- Complete timely and accurate electronic documentation\n
- Collaborate with interdisciplinary healthcare teams to provide holistic community health services\n
- Participate in scheduled weekend rotations\n
