Key Responsibilities
- Oversee and coordinate the RAI/MDS process, including assessments, care planning, triggers, and CAAs
- Manage PDPM/Medicare & Medicaid assessment schedules and deadlines
- Lead and support person-centered care planning across interdisciplinary teams
- Conduct training and audits related to compliance and documentation
- Maintain Medicare certification records and support reimbursement accuracy
- Ensure compliance with state, federal, and OBRA regulations
Requirements
- Active Registered Nurse license in NH
- RAC-CT certification preferred (or willing to obtain)
- 2+ years of long-term care experience with RAI process knowledge
- Strong organizational, documentation, and communication skills
- Familiar with Microsoft Office Suite (Excel, Word, Outlook, PowerPoint)
- Ability to multi-task in a dynamic, team-based setting
