Job Title: Case Manager
Program: CHAI Services – Skill Building Programs
Location: Greater Boston Area, office locations in Waltham and Canton (community visits required)
Hours: Full-time, Monday – Friday (flexible schedule options available)
Position Summary:
We are seeking a passionate and dedicated professional to join our team as a Case Manager. This unique role provides clinical and vocational support to adults with disabilities, including individuals with autism spectrum disorder (ASD) and other intellectual/developmental disabilities. In this position, you will help clients develop vocational skills, secure and maintain meaningful employment, and navigate the complexities of daily living, including social skills, financial management, and accessing community resources.
Key Responsibilities:
- Integrated Skill Development and Personalized Treatment Planning
- Facilitate skill-building opportunities for clients with autism spectrum disorder and other developmental disabilities, focusing on strengthening key life skills such as financial management, self-advocacy, time management, and social communication.
- Collaborate with clients to design personalized support plans that promote independence in both personal and professional environments, fostering empowerment and self-sufficiency.
- Provide continuous skill-building coaching on workplace behaviors, communication, problem-solving, and teamwork, helping clients develop the competencies necessary for long-term employment success.
- Employment Coaching & Job Placement
- Guide clients through the job search process by teaching effective resume writing, job application techniques, and interview skills, while building their confidence in professional settings.
- Cultivate and maintain partnerships with employers to create inclusive employment opportunities that align with clients’ strengths, building essential skills to match workforce demands and ensuring a successful transition into the workplace by providing intensive initial on job support and continuing long term check ins and communications with employers.
- Resource Access and Care Coordination
- Empower clients to access vital resources like healthcare, housing, and SNAP benefits, building their capacity to advocate for themselves and manage their own needs.
- Coordinate with external agencies, family members, and JF&CS programs to ensure clients are equipped with the skills and support they need to independently thrive in the community.
- Provide support during moments of crisis, equipping clients with problem-solving and coping strategies that promote resilience and empower them to overcome challenges in both work and life.
Qualifications:
- Education: Bachelor’s degree in Human Services, Psychology, Social Work, Education, or related field (required). Master’s degree preferred.
- Experience:
- Experience in vocational support or employment services for individuals with disabilities (preferred).
- Experience working with individuals on the Autism Spectrum or with other developmental disabilities (required).
- Experience in case management, counseling, or social services (required).
- Skills:
- Strong interpersonal and communication skills (both verbal and written).
- Ability to work independently, manage a flexible schedule, and meet client needs with minimal supervision.
- Knowledge of Microsoft Office Suite and familiarity with disability services (DDS, SSI, SSDI).
- Compassionate and patient, with a strong desire to help clients succeed in both employment and life skills.
- Additional Requirements:
- Driver’s License & Vehicle: Must have a reliable vehicle and valid driver’s license to transport clients to community visits and employer sites (mileage reimbursed).
- CPR/First Aid Certification: Required or must be able to obtain upon hire.
