Summary: The Direct Support Professional 2 will support and assist in Level 2 residential programs. &nbsp; &nbsp;Participates in agreement meetings annually and assures documentation is updated.Admissions·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Serves as the primary contact for new admission inquiries.·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Completes pre-admission assessment and determination of financial eligibility in collaboration with the Special Care Residence Manager including mental health history and psychosocial issues.·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Assists the Executive Director in implementing marketing plans including networking with potential referral sources such as physicians, hospitals, case managers and other community representatives.&nbsp;·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Educates and assists potential residents and their families with Admissions process, including, but not limited to reviewing lease, rights and disclosures, and ensuring all necessary admission paperwork is completed and maintained.&nbsp;·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Provides tours of the facility, answers all admission calls, and verifies all insurance and financial information. &nbsp;·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Represents the Facility when working with other agencies, departments, and other organizations.&nbsp;Case Management·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Ensures effective case management and follow-up practices.&nbsp;·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Assists residents and families with financial, social security, Medicare and insurance issues as requested.&nbsp;·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Assists Executive Director with regard to policies and procedures, resident safety, ability to age in place, and discharge planning as needed.&nbsp;·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Attends and completes all annual in-services as required by EOEA.&nbsp;·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Participates in multidisciplinary care plan meetings as needed, identifying problems, setting goals and reviewing outcomes.·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Documents all case management activities promptly in Resident records.·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Assists in developing a Comprehensive Service Plan and review every 6 months.·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Reviews Initial Assessments after 30 days and at a minimum of every 6 months thereafter.·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Oversees the completion of Introductory Visits on all new residents and after a significant change.·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Completes all required documentation within regulatory guidelines.·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Provides information and referral as needed for Residents.·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Provides outreach and remains updated on community resources.·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Provides and/or arrange in-services to assist staff in understanding assisted living, Resident’s rights, psychosocial issues, mental health, disabilities, dementia care, and MR.·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Respects the confidentiality and privacy of Residents in and out of building.&nbsp;·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Reports incidents within appropriate time to the Executive Director, and State agency as required by regulations.&nbsp;&nbsp;Employee Management·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Supervises the performance of the licensed and non-licensed nursing staff.&nbsp; Initiates the disciplinary process as outlined in the Rubin Home Assisted Living employee handbook.&nbsp; &nbsp;·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Attends conferences with other professionals and paraprofessionals to assure current standards of practice are maintained within the facility. &nbsp;·&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Participates in On Call rotation.&nbsp;
