ESSENTIAL FUNCTIONS:

1. Provides psychosocial assessments and serves as a resource for home care and hospice patients and their families
2. Conducts bereavement assessments
3. Assesses family/patient needs in preparation for patients death and provides appropriate resources and referral information
4. Assesses family/patient financial needs and refers to the appropriate financial resources as needed
5. Assesses/identifies patient and family risk factors
6. With the family, develops a social work plan of care based on assessment and findings and revises as needed
7. Provides in-service education to staff and volunteers as appropriate
8. Facilitates family/team communication
9. Timeliness of documentation
10. Documentation, visit schedules, and plan of care reflect Medicare skilled criteria and medical necessity to ensure reimbursement
11. Maintains standards of social work excellence in accordance with appropriate regulatory bodies and agency policy
12. Develops and maintains contact with appropriate community agencies and services to promote interagency cooperation and to facilitate related referrals.
13. Assumes responsibility for own personal and professional development
14. Participates in QA and other projects as requested
15. Is an active participant in meetings and in-services
16. Prepares information for patient-focused meetings
17. Adherence to the Social Work Field Education Core Competencies and Practice Behaviors
18. Works in collaboration with the interdisciplinary team
19. Participates in discharge planning
20. Responsible for discussing with manager the distribution of tasks and giving a report prior to taking time off or going on leave. Report will be done both verbally and in writing so that all tasks and responsibilities needing coverage are clear and concise prior to the leave
** Frequent local travel is expected for this position.
OTHER QUALIFICATIONS:
1. Masters degree in social work or bachelors degree in social work from an institution accredited by the Council on Social Work Education or a bachelors degree in psychology, sociology, or other field related to social work
2. Previous social work experience
3. Excellent communication skills necessary
4. Must pass a criminal, driving record check as well as Bureau of Elderly and Adult Services
5. Must have reliable transportation, a valid drivers license, and be able to supply proof of car insurance in the amount of $100,000/$300,000 coverage.
