As a Homecare Licensed Practical Nurse (LPN), you will provide hands on, direct nursing care in accordance with the plan of care. The Licensed Practical Nurse (LPN) will work collaboratively with members of the team to include attending and participating in Case Conferences to help meet positive patient care outcomes. The Licensed Practical Nurse (LPN) will provide meaningful education to patients and support RN Case Managers.

Duties Highlights for a Homecare Licensed Practical Nurse (LPN):
- Assess and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care
- Implements plan of care initiated by the RN Case Manager
- Provides accurate and timely documentation consistent with plan of care
- Provides direct patient care as defined in the State Nurse Practice Act
Qualifications for Homecare Licensed Practical Nurse (LPN):
- Current NH LPN license
- Minimum 2-5 years nursing experiences. Community/home health or medical/surgical experience is preferred
- Proof of Flu vaccination
- Reliable transportation
- Valid driver's license
- Automobile insurance
- Physical exam within the last year
- Two-step TB test
