Confluence is a short-term residential mental health program providing treatment to young adults with psychological, emotional, substance-use and relational challenges. Confluence is a clinically-intensive therapeutic program with a focus on whole person health, including nutrition, physical health and wellness. Confluence is seeking an experienced Registered Nurse to join our clinical team to support the health and recovery of the program participants.
Job Description: Clinical RN and Mindfulness Coach
As part of the Confluence direct-care team, the Clinical Nurse plays an important role overseeing and maintaining the physical health of participants. The Clinical Nurse is an integral link in the spectrum of services offered at Confluence. This person works directly with participants to help maintain wellness and address health concerns as they arise. The Clinical Nurse is crucial in leading and teaching mindfulness based practices daily (Monday - Thursday), including a Mindfulness-Based Stress Reduction class and Yoga. The Clinical Nurse also works as a member of the treatment team working closely with direct-care staff, therapists and clinical director and the health services director.
Conducts initial Nursing Assessments on all new participants and initiates a plan of care
Conducts bi-weekly health reviews for participants, maintaining data on biometrics and documents any reported symptoms or health concerns
Manage daily medication assistance processes assuring that all participant medications are correctly packaged, available, and appropriately documented and accounted according to regulations and policies and procedures
Liaises with outside physicians/pharmacies to manage prescriptions and the physical health of program participants
Trains and assists staff who provide medication assistance
Liases with Clinical Team to address concerns regarding participants physical health
Provides monitoring and education to participants on best practices to improve nutrition, sleep, activity and weight management
Leads one Yoga class per week.
Leads a weekly 1.5 hour long Mindfulness-Based Stress Reduction group
Facilitated daily mindfulness practices with participants, including 1 hour/day of guided meditation
Makes appropriate referrals to outside health professionals for any health conditions
Qualifications, Skills and Experience
Registered Nurse with current Vermont License
Experience in a nursing position in an acute, ambulatory, mental health, school or long term care setting
Demonstrated competence in nursing assessment, medication administration, and patient education
Psychological/psychiatric training or experience a strong plus
Interest in working with young adult populations a plus
Effective interpersonal skills and ability to work independently
Experience in teaching yoga and mindfulness practice
Ability to guide group meditation practice
Interest and experience in Mindfulness-Based Stress Reduction (MBSR)
Training in Mindfulness-Based Stress Reduction (MBSR) preferred
Experience with electronic medical records
Must be able to pass background check
Competitive hourly pay rate
Full-time (40 hours) work schedule
